Central Vermont Medical Center follows every state and federal rule to protect your privacy and limit any unwanted access to your health information. This information belongs to you. It cannot be released to anyone, including your relatives, unless you agree.
When you are admitted as a patient to CVMC, you will be asked to sign a treatment consent form that outlines the conditions under which we can provide care for you. This form contains:
- Consent to Admission and Medical Treatment
- Consent to Release of Information
- Consent to Specimen Use
- Acknowledgment of receipt of CVMC's Patient Bill of Rights
- Consent that CVMC is not responsible for patient valuables
The consent form cannot be altered in any way. If you request changing the words or if you refuse to sign the form, Central Vermont Medical Center cannot proceed with your treatment or testing.
In order to be a patient, you must sign this form.
