Central Vermont Medical Center's MyHealth Online are portals that allow patients to access their personal health record as well as test results and other information from where ever you have access to the intranet.

CVMC's Patient Portals are online tools that help you to manage your healthcare easily and securely, allowing you to view your personal health record whenever and where ever you have access to the intranet.

CVMC has two separate patient portals that you can access, depending on the service you are using:

IMPORTANT NOTICE - Effective November 9, 2019

On November 9, your outpatient portal changed to MyChart. All users need to create a new account. (Note: If you already have a MyChart account with the UVM Medical Center you can use the same login.)

You can sign up for MyChart in two ways: Using an activation code or completing an online web form.

Use an Activation Code

If you have an activation code from your provider’s office, a recent bill, or a printed After Visit Summary, visit MyChart.UVMHealth.org/Code

On the Enter Your Activation Code page, enter your activation code and your date of birth. Click Next. For next steps, see "Account Information and Preferences" below.

If you do not have an activation code, click the button below to compete an online sign-up form.

On the Create a MyChart Account page, fill out the secure form and click Submit Request.

Upon submission, you will be sent an activation link by email. Clicking this link from your email returns you to the Use an Activation Code page and automatically inputs the code. Once the code has been entered, confirm your birthday and click Next.

Enter Account Information and Preferences

Choose a Username & Password: Fill out the account information page with your desired username, password, and security question. Accept the terms and conditions by clicking Accept at the bottom of your screen.

Update Your Communications: Choose your preferred communication methods for MyChart communications and alerts.

Portal FAQs

Where does my health information in the Portal come from?

All of the information in the Portal comes from your Central Vermont Medical Center Electronic Health Record. This ensures that you have access to the most accurate, up-to-date information possible.

If I already signed up for a portal to access information from a CVMC Medical Group Practice provider, do I have to sign up again to access information from my hospital stay? Or vice versa?

Yes. These are two different sets of records so you do need to sign up for each separately.

How do I log into the Portal?

To log into the Portal, use the Patient Portal link listed above for the portal you need to login to, or follow the sign up instructions for the portal you want to create an account for.

Is my information safe?

Yes. Portal passwords are encrypted and URLs are rewritten so that they cannot be copied and pasted. You and authorized family members are the only ones who can access your Portal. Also, a timeout feature protects your information if you leave the Portal page open.
