What Makes Our Benefit Program Special?

Our employee medical plan premiums have only increased once in the past four years, primarily because of our transition to a self-funded plan four years ago and the implementation of an excellent wellness program. We also established two Express Care locations as an affordable alternative to the Emergency Room for urgent but not life threatening emergency medical needs outside of primary care hours.

CVMC’s Overall Benefit Contribution = 30% of salary on average  

CVMC provides a comprehensive benefit package to full time and part time employees:

Full Time Employee: 72 – 80 hours per bi-weekly pay period.
Part Time Employees: 40 – 71 hours per bi-weekly pay period.

Medical Plan

Choice of HMO or PPO plan. Both plans include hospital/physician services and provide prescription drug coverage (including our in-house pharmacy benefit). CVMC pays 64% – 82% of premium cost.

Dental Plan

100% coverage on preventative care, up to 4 cleanings per year allowed. Basic restorative coverage at 80%. Major restorative and orthodontics covered at 50%. Annual limit $1,500.00 per person.Separate $1,500.00 lifetime limit on Orthodontics. CVMC pays 50% - 65% of premium cost.

Vision Plan

Plan includes exam and lenses each year, frames every other year, subject to plan maximum. $10 co-pay on exam, $25 co-pay on materials. Additional discounts available.

Life Insurance

100% employer paid term life benefit equal to 1.5 times annual base salary. Additional term life benefits available including a valuable yet inexpensive dependent life insurance for your family.

Disability Insurance

100% employer paid long term disability benefit equal to 60% of monthly base salary, up to $13,000.  180 day elimination period.  Additional CVMC subsidized long term and short term disability options available.

Medical & Dependent Care Reimbursement

Voluntary pre-tax contributions for out of pocket medical expenses and dependent care expenses. IRS maximums apply.
