Job Description

POSITION SUMMARY

Under the direction of the Director of Care Management, the Care Coordinator is responsible for coordination and management of the hospital based population.  This work could include coordinating care and service of a selected patient population, promoting effective utilization and monitoring of health care resources, providing guidance to the interdisciplinary team to achieve desired clinical, financial and resource outcomes.  Provides employee assistance, consultation to staff, physicians, and community providers regarding psychosocial aspects of care.

MINIMUM REQUIREMENTS

  • Preferred MSW or BSN RN with acute healthcare setting experience, MA in related field acceptable with acute healthcare experience.

  • Will consider experienced LPN/RN with acute and/or Skilled Nursing Facility healthcare setting experience, or Bachelor’s level working towards a Master’s level degree in Social Work or Counseling.

EXPERIENCE

  • 3-5 years of clinical experience. 

  • Fulfills annual hospital mandatory requirements for safety, MSDS, infection control. 

  • Demonstrates leadership qualities.
