100% coverage on preventative care, up to 4 cleanings per year allowed. Basic restorative coverage at 80%. Major restorative and orthodontics covered at 50%. Annual limit $1,500.00 per person.Separate $1,500.00 lifetime limit on Orthodontics. CVMC pays 50% - 65% of premium cost.
